Home-Based and Community Assistance Programs

WHAT IS HCS?

In order to enable Texans with an intellectual disability (ID) or a related condition to live in the community, HCS, a Medicaid waiver program, provides services and supports. HCS services are meant to complement, not replace, services obtained from natural supports like family, neighbors, or community organizations, or from other programs like Texas Health Steps.

AREAS THAT WE COVER

The entire state of Texas is served by Maggie’s Divine Homes from Dallas, Fort Worth, Austin, San Antonio, Houston, and Laredo to Denton, Gainesville, and Sherman. We are ready and committed to helping people and families throughout the entire state, and we speak both Spanish and English.

WHO PROVIDES HCS SERVICES

To deliver HCS services, the Texas Health and Human Services Commission (HHSC) enters into agreements with both public and private organizations. To guarantee the caliber of services, HHSC conducts routine monitoring. Applicants select their provider from a list of local providers when they enroll in the HCS program. People can switch providers whenever they want, even if they relocate to a different part of Texas.

WHO CAN GET HCS?

Any Texas resident who does not reside in an institutional setting may be eligible for HCS if they:

  • Possesses an authorized linked ailment with an IQ of 75 or lower, or has an IQ of 69 or lower.
  • Has minor to severe deficiencies in adaptive behavior.
  • Is qualified to get Medicaid assistance.
  • Does not have an active Medicaid waiver enrollment.
We can also provide personal support services, habilitation or emergency response services through the Community First Choice (CFC) program.
OUR SERVICES

Available Services Through the HCS Waiver

ADAPTIVE AIDS

Adaptive aids are appliances, controls, or gadgets that help people maintain or improve their capacity to carry out daily tasks or manage their surroundings. In addition to helping with movement and communication, adaptive aids can be used to treat, prevent, or compensate for conditions that cause impairment or loss of function.

BEHAVIORAL SUPPORT

Behavioral support provides specialized interventions that help people overcome behaviors that prevent or interfere with home, family, and community life. These strategies are personalized to promote positive behavior change and enhance overall quality of life.

Community-Based Support Services

Community First Choice Services helps people living on their own or in their families’ homes to learn, retain or improve skills related to activities of daily living. This includes personal grooming, cleanliness, bed making and household chores, preparing and eating meals, and social and adaptive skills. Services may include protective supervision and help with performing health related tasks that are delegated by a registered nurse.

DAY HABILITATION

In order to successfully live in the community, people can learn, retain, and enhance their self-help, socialization, and adaptive skills at Day Habilitation. This supportive environment fosters personal growth, independence, and meaningful daily engagement.

NUTRITION SERVICES

Our nutritional services are delivered by a certified dietician who conducts in-person staff training, consultations, and assessments. Determining basic or specific therapeutic nutritional needs is one of the services offered.

SPEECH THERAPY

Corrective or rehabilitative treatment for those with speech, hearing, or language problems is included in speech therapy. Among the services offered are needs assessments, treatment plan development, therapeutic intervention determination, training, assistance with adapted aids, and provider and family consultation.

RESIDENTIAL SUPPORT SERVICES

Group home supervision and help are part of residential support services. During regular sleeping hours, our service provider employees stay up late. In order to treat medical illnesses, manage maladaptive behaviours, and help with everyday life activities, the majority of people need residential support services.

PHYSICAL THERAPY

Physical therapy supports individuals in enhancing their mobility, physical function, and overall physical fitness. Services involve evaluating the person’s needs, creating a treatment plan, selecting appropriate therapeutic interventions, providing training, assisting with adaptive equipment, and offering guidance to both the family and care providers.

RESPITE CARE

This service offers short-term support to give unpaid primary caregivers a break or to provide care during their absence. Respite care can take place either at home or in another setting. It is available to individuals enrolled in the CFC program.

SUPPORTED EMPLOYMENT

Supported employment assists individuals with disabilities in maintaining paid jobs by providing the intensive, ongoing support they need to succeed in the workplace. These services must be delivered in integrated settings where individuals with and without disabilities work together. Support may include job adaptations, supervision, and training tailored to the person’s specific diagnosis.

OCCUPATIONAL THERAPY

Occupational therapy offers specialized care to support individuals in achieving independence in daily activities. Services include evaluating the individual’s needs, creating a personalized treatment plan, selecting appropriate therapeutic interventions, providing training, assisting with adaptive equipment, and working closely with the family and care providers.

NURSING SERVICES

Nursing services are delivered by a licensed nurse and include monitoring health status, administering and overseeing medications, offering referrals to medical professionals, and providing training to individuals, family members, and support staff.

Foster Home or Companion Support Services

We offer individuals of all ages a home-like setting as an alternative to residing in a group home (previously known as foster care).

Job Support Services

Employment assistance helps individuals secure paid jobs within the community by identifying their interests, assessing their skills and support requirements, and connecting them with suitable employers.
